<p><strong>The Discharge Gap is a Real, Researched Problem</strong></p>
<p>Researchers in Ireland have studied this exact issue. A major Health Research Board (HRB) programme called iPASTAR found that many patients and families experience fragmented care, difficulties when they were discharged, and a lack of access to ongoing therapy.</p>

<p><strong>The National Plan to Fix It: More Care at Home</strong></p>
<p>The good news is that the HSE has a plan to fix this. The <strong>National Stroke Strategy 2022-2027</strong> is investing millions of euros to rapidly expand <strong>Early Supported Discharge (ESD) teams</strong>.</p>
<p>This means that instead of staying in hospital, you will be discharged earlier and your specialist therapy team (physiotherapists, OTs) will visit you <em>at home</em>.</p>
<p>This is a fantastic development, but it puts a new responsibility on the patient: to continue the hard work <em>between</em> those therapist visits.</p>

<p>It&#8217;s not just a document for hospitals. It has a direct impact on you and your family. So, what’s changed?</p>
<ol>
<li><strong>The New Mandate: More Intensive Rehab</strong></li>
</ol>
<p>The evidence is clear: to get the best results, rehabilitation needs to be intensive. The days of gentle, infrequent exercise are over. The new guideline calls for more focused, high-dosage therapy to help the brain rewire (a process called neuroplasticity).</p>

<li><strong>The New Focus: Self-Directed Therapy</strong></li>

<p>For the first time, the 2023 Guideline includes a new section on self-directed therapy. This means you, the patient, are encouraged to take an active, empowered role in your own recovery at home. Your therapy team will guide you, but the work you do between their visits is now recognised as a critical part of the plan.</p>
<p><strong>The Challenge: How Do You Do Intensive Rehab by Yourself?</strong></p>
<p>This is the key question. If you have limited mobility, how can you safely perform intensive, repetitive exercises on your own?</p>
<p><strong>The Solution: Technology Designed for the New Standard</strong></p>
<p>This is exactly why the Tigo 558 was developed. It&#8217;s designed to help you meet the guideline from your own home.</p>
