The Discharge Gap is a Real, Researched Problem

Researchers in Ireland have studied this exact issue. A major Health Research Board (HRB) programme called iPASTAR found that many patients and families experience fragmented care, difficulties when they were discharged, and a lack of access to ongoing therapy.

The National Plan to Fix It: More Care at Home

The good news is that the HSE has a plan to fix this. The National Stroke Strategy 2022-2027 is investing millions of euros to rapidly expand Early Supported Discharge (ESD) teams.

This means that instead of staying in hospital, you will be discharged earlier and your specialist therapy team (physiotherapists, OTs) will visit you at home.

This is a fantastic development, but it puts a new responsibility on the patient: to continue the hard work between those therapist visits.
